Impingement syndrome: Similar to rotator cuff tendonitis, impingement syndrome may present with the same symptoms, but from a different cause. Impingement syndrome occurs when tendons are pinched between two structures that perform rotating functions in the shoulder (bone, etc.).  

Bicipital tendonitis: Located in the upper portion of your arm, the bicep connects to the front and side of the shoulder. If inflamed, the bicep can generate pain in the shoulder or pain in your forearm and/or elbow (radiating pain). This pain typically causes those with bicipital tendonitis to compensate, straining the back/neck until tendonitis forms in that area too.

Cervicogenic headache: While not exactly tendonitis. Inflammation of the muscles around the spine can cause referred pain in other areas of your body. If located in the upper portion of the spine, tendonitis may create a headache , the same nerves affected by upper spine tendonitis handle the head as well.

Causes of Neck/ Back Tendonitis  

Those with rheumatoid arthritis, gout, psoriasis, thyroid disease , and infection are more likely to develop tendonitis of the back or neck. Athletes, those who do manual labor, people who have experienced trauma (like whiplash), people with incorrect posture and people who work on the computer are all at risk.
